目的 采用孟德尔随机化分析方法,探讨肠道菌群与糖尿病肾病的因果关联。方法 应用两样本孟德尔随机化(MR)方法,基于最大的公开全基因组关联研究(GWAS)汇总数据,采用逆方差加权法 (IVW)、加权中位数法(WM法)、MR-Egger回归法、MR多效性残差及异质性检测法(MR-PRESSO)进行分析。结果 在IVW方法中,拟杆菌门(OR=1.43,95%CI:1.17~1.74),拟杆菌纲(OR=1.45,95%CI:1.21~1.74),拟杆菌目(OR=1.45,95%CI:1.21~1.74),铁红隐杆菌属(OR=1.16,95%CI:1.06~1.28),链杆菌属(OR=1.31,95%CI:1.20~1.4),毛囊梭菌属(OR=1.43,95%CI:1.17~1.76)以及副萨特氏菌属(OR=1.27,95%CI:1.13~1.43)与糖尿病肾病呈正相关,未检测到显著的异质性或多效性。结论 本研究发现7个肠道细菌分类群与糖尿病肾病风险增加有关,证实了肠道微生物群在DN的发病机制中起重要作用,可为糖尿病肾病的筛查和预防提供了新的潜在指标和靶点。 |

Objective To explore the causal relationship between intestinal flora and diabetes nephropathy by Mendelian randomized analysis. Method Two samples of Mendelian randomization (MR) were applied,and based on the largest publicly available genome-wide association study (GWAS) summary statistics,inverse variance weighted (IVW),weighted median (WM),MR Egger regression,Mendelian randomization pleiotropy residual sum and outlier (MR-PRESSO) were used for bidirectional Mendelian randomization (MR) analysis of the two samples. Result In the IVW method,phylum.Bacteroidetes (OR=1.43,95%CI:1.17~1.74),class.Bacteroidia (OR=1.45,95%CI:1.21~1.74),order.Bacteroidales (OR=1.45,95%CI:1.21~1.74),genus.Candidatus Soleaferrea (OR=1.16,95%CI:1.06~1.28),genus.Catenobacterium (OR=1.31,95%CI:1.20~1.4),genus.Lachnoclostridium (OR=1.43,95%CI:1.17~1.76),and genus.Parasutterella (OR=1.27,95%CI:1.13~1.43) was positively associated with diabetic nephropathy,and no significant heterogeneity or pleiotropy was detected. Conclusion We found that seven intestinal bacterial groups were related to the increased risk of diabetes nephropathy,which confirmed that intestinal microflora played an important role in the pathogenesis of diabetes nephropathy.Our work also provides new potential indicators and targets for the screening and prevention of DN. |
