| 随着慢性病发病率的持续攀升及患者生存期的显著延长，慢性病患者的社会融合问题日益成为公共卫生与临床相关领域关注的重点。本文综述慢性病患者社会融合的概念、评估工具、影响因素及干预措施，旨在为开发本土化慢性病患者社会融合评估工具、制定多学科协作干预方案，以及提升慢性病患者融合水平和生活质量提供参考。 |

| With the continuous rise in the incidence of chronic diseases and the significant extension of patients’ survival periods, the issue of social integration among patients with chronic diseases has increasingly become a focus of attention in public health and clinical-related fields. This article reviews the concept, assessment tools, influencing factors, and intervention measures of social integration for patients with chronic diseases, aiming to provide references for developing localized social integration assessment tools for patients with chronic diseases, formulating interdisciplinary collaborative intervention plans, and improving the integration level and quality of life of patients with chronic diseases. |
