| With the continuous increase in the incidence of chronic diseases and the significant extension of patients' survival periods,social integration among patients with chronic diseases increasingly become a focus of attention in public health and clinical-related fields.This article reviews the concept,assessment tools,influencing factors,and intervention strategies of social integration for patients with chronic diseases,aiming to provide references for developing localized social integration assessment tools for patients with chronic diseases,formulating interdisciplinary collaborative intervention plans,and improving the integration level and quality of life of patients with chronic diseases. |
