      创伤后应激障碍(post-traumatic stress disorder,PTSD)是严重创伤事件后常见的慢性精神疾病,其发病机制复杂,涉及神经生物学、免疫系统及遗传表观调控等多维度的交互作用。在下丘脑-垂体-肾上腺轴功能失调被视为多种致病机制共同通路的研究背景下,本文综述了严重创伤患者PTSD发病机制的研究进展。功能异常的下丘脑-垂体-肾上腺轴通过调控皮质醇分泌、糖皮质激素受体敏感性及与交感神经、炎症反应的交互作用,在PTSD发病中起关键作用。同时,肠道菌群紊乱、神经环路异常及个体遗传多态性通过影响下丘脑-垂体-肾上腺轴加剧疾病进展。本文系统梳理了上述机制间的内在联系,以期为未来PTSD的相关研究及预防治疗提供新的思路与参考。
英文摘要:
      Post-traumatic stress disorder is a common chronic mental illness following severe traumatic events,with complex pathogenesis involving multi-dimensional interactions such as neurobiology,immune system,and genetic epigenetic regulation.Against the background that hypothalamic-pituitary-adrenal axis dysfunction is regarded as a common pathway for various pathogenic mechanisms,this article reviews the research progress on the pathogenesis of post-traumatic stress disorder in patients with severe trauma.This study found that the dysfunctional hypothalamic-pituitary-adrenal axis plays a key role in the pathogenesis of post-traumatic stress disorder by regulating cortisol secretion,glucocorticoid receptor sensitivity,and its interactions with the sympathetic nervous system and inflammatory responses.Meanwhile,intestinal flora dysbiosis,abnormal neural circuits,and individual genetic polymorphisms exacerbate disease progression by affecting the hypothalamic-pituitary-adrenal axis.This article systematically sorts out the internal connections among the above mechanisms,aiming to provide new ideas and references for future post-traumatic stress disorder-related research, prevention,and treatment.
