      目的 探究烟台市开展DRG省级付费试点后,各级医疗机构住院患者的就医满意度及其影响因素。方法 采用问卷调查法对烟台市不同级别医疗机构的住院患者展开满意度调查,运用描述性分析、卡方检验和二元logistic回归分析对170份有效问卷进行分析。结果 烟台市公立医院住院患者总体满意度为90.58%。医患沟通与医德医风维度得分较高,医疗费用与医疗服务维度得分偏低。多因素分析显示,付费方式(P=0.001)中城镇居民医疗保险(OR=95.240,95%CI: 7.692~1179.285)和新型农村合作医疗(OR=83.876,95%CI: 3.790~1856.032),以及初次就诊时间(P<0.001)中2020–2021年(OR=0.006)、2022–2023年(OR=0.025)和2024年(OR=0.052)均为患者满意度的独立影响因素。结论 烟台市应持续深化医药卫生体制改革,优化医疗机构管理路径,着力改善就医服务、环境与模式,切实提升住院患者满意度和医疗服务质量。

      Objective To explore the satisfaction of hospitalized patients in medical institutions at all levels and its influencing factors after the implementation of DRG provincial payment pilot in Yantai. Methods A questionnaire survey was conducted to investigate the satisfaction of inpatients in different levels of medical institutions in Yantai. Descriptive analysis, chi square test and binary logistic regression analysis were used to analyze 170 valid questionnaires. Results The overall satisfaction of inpatients in public hospitals in Yantai was 90.58%. The scores of doctor-patient communication and medical ethics were higher, while the scores of medical expenses and medical services were lower. Multivariate analysis showed that medical insurance for urban residents (OR=95.240, 95% CI: 7.692-1179.285) and new rural cooperative medical system (OR=83.876, 95% CI: 3.790-1856.032) in payment mode (p=0.001), and the first visit time (p<0.001) in 2020-2021 (OR=0.006), 2022-2023 (OR=0.025) and 2024 (OR=0.052) were independent influencing factors of patient satisfaction. Conclusion Yantai should continue to deepen the reform of the medical and health system, optimize the management path of medical institutions, focus on improving medical services, environment and mode, and effectively improve the satisfaction of hospitalized patients and the quality of medical services.
