| 目的 探讨基于个案管理模式的出院计划对乳腺癌化疗患者的干预效果。方法 选取山东省济宁市某三甲医院乳腺中心2023年4月到2024年3月收治的160例乳腺癌术后化疗患者，随机分为干预组和对照组各80例，对照组实施常规护理和出院指导，干预组实施基于个案管理模式的出院计划。比较两组患者的出院指导质量、出院准备度、自我管理效能、30d非计划再入院率。结果 干预组出院指导质量、出院准备度、自我管理效能均高于对照组（P<0.05），30d非计划再入院率低于对照组（P<0.05）。结论 基于个案管理模式的出院计划，有助于提高乳腺癌术后化疗患者的出院指导质量、出院准备度及自我管理效能，降低30d非计划再入院率。 |

| Objective To explore the intervention effect of discharge plan based on case management model on breast cancer patients undergoing chemotherapy.Method 160 breast cancer patients who received chemotherapy after surgery in a tertiary A hospital in Jining Shandong Province from April 2023 to March 2024 were selected and randomly divided into an intervention group and a control group with 80 cases each. The control group implemented routine nursing intervention and discharge guidance, and the intervention group implemented the discharge plan intervention scheme based on the case management model. Compare the discharge readiness, quality of discharge guidance, self-management effectiveness, and 30 day unplanned readmission rate between two groups of patients.Results 80 cases in the intervention group and 80 cases in the control group completed the study. The intervention group had significantly better discharge readiness, discharge guidance quality, self-management efficiency and less 30 day unplanned readmission rate than the control group (P<0.05).Conclusion The discharge plan based on case management mode is helpful to improve the discharge preparation, discharge guidance quality and self-management efficiency of breast cancer patients receiving chemotherapy after surgery, and reduce the 30 day unplanned readmission rate. It is worth popularizing in clinical practice. |
