| 目的 探讨采用在黏膜下注射生理盐水，辅助支撑喉镜下低温等离子射频消融术切除声带白斑的临床疗效。方法 选取2016年2月～2019年10月期间入院诊治的43例声带白斑病例作为研究对象，随机分配观察组（n=24）及对照组（n=19），对观察组患者施行黏膜下注射生理盐水辅助低温等离子消融手术切除白斑，对照组患者实施传统声带黏膜剥脱术治疗。术后随访研究两种方法的临床疗效。结果 观察组平均手术持续时间6.74±2.45min，对照组平均手术持续时间15.11±2.66 min；观察组术中出血量平均为1.76±0.56ml，对照组术中出血量平均为4.17±1.63ml；观察组术后NRS疼痛评分0.89±0.19分，对照组术后NRS疼痛评分2.57±0.85分；观察组VHI主观评分49.42±4.87分，对照组VHI主观评分52.68±4.91分；观察组嗓音恢复较对照组好，观察组3年复发率较对照组低。以上各组差异均具有统计学意义（P＜0.05）。结论 通过在黏膜下注射生理盐水的手段创新，辅助支撑喉镜下低温等离子对声带白斑治疗更加微创、术后舒适、嗓音恢复好，该手术方法更加完善，值得临床推广。 |

| Objective To investigate the clinical effect of cryogenic plasma assisted by submucosal saline injection under laryngoscopy to remove vocal cord leukoplakia. Method 43 cases with vocal cord leukoplakia in hospital during Feb.2016 to Oct.2019 were selected, randomly assigned to observation group (n = 24) and control group (n = 19). Patient’s vocal cord leukoplakia in the observation group was surgically removed by low-temperature plasma radio frequency ablation assisted by injection of normal saline into submucosal, while that of the control group was treated with traditional vocal cord mucosal stripping. Postoperative follow-up investigated the clinical efficacy of both methods. Results The average duration of surgery in the observation group was 6.74±2.45min, while that of the control group was 15.11±2.66 min. The average intraoperative blood loss in the observation group was 1.76±0.56 ml, while the average intraoperative blood loss in the control group was 4.17±1.63 ml. Postoperative NRS pain score was 0.89±0.19 in the observation group and 2.57±0.85 in the control group. The subjective VHI score was 49.42±4.87 points in the observation group and 52.68±4.91 points in the control group. The sound recovery of the observation group was better than that of the control group, and the 3-year recurrence rate of the observation group was lower than that of the control group.The differences in all of the above groups were statistically significant (P < 0.05). Conclusion The innovation of cryogenic plasma assisted by submucosal saline injection to resect vocal cord leukoplakia is more minimally invasive , more comfortable after surgery and better voice recovery for patients, so this surgical method is more perfect and worthy of clinical promotion. |
