      目的 分析红霉素和阿奇霉素对肺炎支原体感染儿童血清细胞因子和肺功能的影响,评价其疗效差异。方法 将2017年9月至2020年9月芜湖市第二人民医院儿科收治的100 例肺炎支原体肺炎患儿随机分为红霉素组(n=49)与阿奇霉素组(n=51),在常规治疗基础上分别使用红霉素和阿奇霉素进行抗感染治疗,通过比较两组患儿治疗后临床疗效、不良反应发生率、血清炎性细胞因子水平和肺功能的差异,评价红霉素和阿奇霉素的治疗效果。结果 红霉素组临床治愈、好转和无效的例数分别为18例、20例、11例,阿奇霉素组分别为25例、24例、2例,两组治疗效果差异有统计学意义(χ2=7.698,P=0.021)。治疗后两组TNF-α、IFN-γ、IL-4 与Th1/Th2均降低(均P<0.05),肺功能指标最大呼气中段流量(MMF) 、峰流速(PEF) 、最大吸气压(MIP) 及最大呼气压(MEP) 均升高(均P<0.05),且阿奇霉素组变化较红霉素组更明显,差异有统计学意义(均P<0.05)。两组间不良反应发生率的差异无统计学意义(P>0.05)。结论 阿奇霉素对儿童支原体肺炎的疗效优于红霉素。

      Objective To analyze the effects of erythromycin and azithromycin on serum cytokines and pulmonary function in children with mycoplasma pneumoniae infection,and to evaluate the difference of therapeutic effects.Methods A total of 100 pediatric inpatients with mycoplasma pneumoniae were selected and randomly divided into two groups:erythromycin group (n=49) and azithromycin group (n=51).On the basis of conventional treatment,erythromycin and azithromycin were used for anti-infection treatment respectively.Therapeutic effect were evaluated through the comparisons of treatment outcomes,adverse reactions,serum levels of inflammatory cytokines and pulmonary function.Results In erythromycin group,18 cases were cured,20 cases were effective and 11 cases were ineffective,while those in azithromycin group were 25 cases,24 cases and 2 cases respectively,and the difference between the two groups was significant (χ2=7.698,P=0.021).After treatment,TNF-α,IFN-γ,IL-4 and Th1/Th2 in the two groups were decreased (all P<0.05),and the pulmonary function indexes such as MMF,PEF,PImax and PEmax were increased (all P<0.05),and all the changes mentioned above were more obvious in azithromycin group (all P<0.05).The incidence of adverse reactions between the two groups were not significantly different(P>0.05).Conclusion The therapeutic effects of azithromycin is better than erythromycin on mycoplasma pneumonia in children.
