| 目的 探讨踇长屈肌腱转位联合腓肠肌腱膜V-Y延长治疗陈旧性跟腱缺损5cm以上的手术方法及效果。方法 应用踇长屈肌腱转位联合腓肠肌腱膜V-Y延长治疗陈旧跟腱缺损5cm以上患者14例,采用采用美国足踝外科协会(American Orthopaedic Foot and Ankle Society,AOFAS)踝与后足评分和视觉模拟法(Visual Analogue Scale,VAS)疼痛评分系统对患者进行评估,通过对患者术前及术后末次随访评分差异进行分析,进而对患者康复及预后进行评价。结果 发现患者术后末次随访AOFAS评分明显高于术前AOFAS评分,差异有统计学意义(P<0.05)；发现患者术后末次随访VAS评分则明显低于手术前VAS评分,差异也有统计学意义(P<0.05)。结论 踇长屈肌腱转位联合腓肠肌腱膜V-Y延长治疗陈旧性跟腱缺损可减少对踇趾功能的影响,避免小腿三头肌进一步萎缩。提高了跟腱修复的强度。 |

| Obiective To explore the surgical methods and effect of the reconstruction of more than 5cm chronic achilles tendon rupture combining flexor hallucis longus tendon transfer and V-Y tendinous flap of gastrocnemius. Method Using AOFAS and VAS to assessment the patients who has operated with reconstruction of more than 5cm chronic achilles tendon rupture combining flexor hallucis longus tendon transfer and V-Y tendinous flap of gastrocnemius. Based on theSpreoperative and postoperativeSfollow-upSscoreSdifference analysis,Sthen to evaluate theSrehabilitationSand prognosis of patients. Result The score of AOFAS of patient at the end of follow-upSafter operation were higher than these before the operation, the difference has statistically significant (P<0.05). The score of VAS of patient at the end of follow-upSafter operation were lower than these before the operation, and the difference also has statistically significant (P<0.05). Conclusion Hallucis long flexor tendon transposition joint calf muscle aponeurosis V-Y extended treatment of chronic Achilles tendon rupture can reduce the influence of hallucis toe function, avoid leg triceps shrink further. Increasing the intensity of the Achilles tendon repair. |
